Becoming a New Patient
 


Dr. Cook and the staff of VitalCare Institute Institute of Health would like to welcome you to our practice. We provide quality women's health care for a wide variety of gynecological conditions. Simply put, we care for our patients the way we would want our loved ones to be treated - with the dignity and respect that you desire. Steps to Become A New Patient
This page details the process of becoming a patient at VitalCare Institute of Health.

Schedule Surgery with Dr. Cook
If you prefer to start the process online, complete the form on this page and submit it to Dr. Cook. Michelle, VitalCare's new patient coordinator will contact you, usually within 24 hours.

VitalCare New Patient Packet
You will need a user ID and a password to access the new patient packet. Michelle will provide you with this information when you speak with her to set up an appointment with Dr. Cook.

Medical Release Form

You will need to have your local doctor send a copy of your medical records to Dr. Cook at VitalCare Institute of Health. Print out the Medical Release form, complete it and send it to your doctor.
